Scarsdale High School

Every 9th and 11th Grade student, prior to school entry in September, is required to have a Physical Exam performed by a M.D., N.P. or P.A. (Out-of-state N.P's and P.A.'s must be countersigned by the overseeing M.D.) A Dental Certificate is requested at this time.

These exams must be current and have been done during the preceding 12 months.  No student will be permitted to participate in PE without this on file.

The following forms may be used by the practitioner:

• Dental Health Certificate Form

The SHS Health certificate form must  be used if the exam is to be used for Interscholastic Sports, aka Gold Card (see "Gold card" page for more details).
