Immunization Requirements

New York State Immunization Requirements

ALL STUDENTS MUST MEET the NYS IMMUNIZATION REQUIREMENTS: 

IMMUNIZATION DOSES ACCEPTIBLE ALTERNATIVES
Diptheria Toxoid 3 doses DPT,Dt,Dtap,TD
Poliovirus vaccine 3 doses OPV,IPV, or elPV
Measles 2 doses The first administered AFTER 12months of age. Most often given as MMR
Mumps 2 doses Administered AFTER 12 months of age . Most often given as MMR
Rubella 1 dose Administered AFTER 12 months of age. Most often given as MMR
Hepatitis B 3 doses Accordance with the ACIP Schedule
Varicella 1 dose None
Tdap 1 dose None
Meningoccocal 12th grade effective 9/16 1 dose Menactra, Menveo

 

 2020-2021 NY Immunization Requirements

Students may attend school if immunizations are in progress. They must schedule the next dose required at the earliest, medically recommended opportunity.

The physician's own form Must be used to document and sign immunizations. 

Serologic evidence is now acceptable for   MMR, Hep B and Varicella.
