Please take a moment to comply with this very important request. 

We highly recommend that you SUBMIT YOUR CHILDS ANNUAL EXAM AND IMMUNIZATION RECORD EACH AND EVERY YEAR.  This allows your child to remain in school,participate in physical education , fulfill necessary mandated screenings and meet immunization requirements.
 
In addition, ALL students who want to participate in our sports program will require a recent physical on file.   The exam must be less than one year old.
 
ALL reports can  be mailed, emailed to the High School nurses at CKroll@scarsdaleschools.org or SCrowley@scarsdaleschools.org or faxed to 721-2443.
